  • 10th März 2026, Lessons Learned: Temporary Platform Collapse - Lift Shafts Feb2026

    Please take a moment to review the SafeWork Alert of formwork structurally collapsing in a lift shaft. This has significant relevance to our scope of works and will be discussed further for prevention measures.

    During building construction temporary working platforms may be required inside lift shafts to facilitate activities such as formwork, reinforcement, or finishing works.

    This type of working platforms fits the definition of void scaffolds as per AS/NZS 1576.1:2019. SafeWork NSW has responded to three incidents involving the failure/collapse of temporary working platforms installed in lift shafts.

    These incidents resulted in workers falling from height and sustaining serious injuries. Temporary working platforms in lift shafts present a high risk due to the potential for sudden failure, limited rescue access, and the significant fall distances involved.

    Incident 1: A temporary steel platform supported on reinforcement bars inside the lift shaft collapsed during formwork activities, resulting in a worker falling approximately 8 metres and sustaining injuries.

    Incident 2: A temporary timber platform inside the lift shaft collapsed during the stripping of formwork, resulting in two workers falling approximately 3.5 metres and sustaining injuries.

    Incident 3: A temporary timber platform inside the lift shaft collapsed, triggering the progressive failure of multi-level temporary platforms below. As a result, a worker fell more than 20 metres and sustained injuries.

  • 26th Februar 2026, Post-Tensioning Coil Injury

    During Strand Pushing Operations the coil was tangled in the backpack. The operator stopped pushing while a worker went to investigate. On identifying the problem the worker attempted to free up the tangled strand and while attempting to untangle it, stored energy was released, the strand flicked making contact with the workers face. The worker was wearing safety glasses at the time however he received a laceration to the bridge of his nose and a laceration to his lip. Following first aid the worker was taken to a medical center where it was confirmed that he has suffered a minor facial fracture. The wound to his lip required several stitches.

  • 26th Februar 2026, Fatality at Losinger Marazzi

    During vertical drilling works using a hydraulic drill, the operator was fatally injured by a tool/item of plant that was reportedly projected from the machine.
    This has prompted a company wide review of ALL drilling operations (piling, tower crane pad drilling) where stored energy or pressurisation is a hazard.
    The review must include:
    - A desktop (risk register and management plan) review of the activities
    - On-site SWMS review
    - Ensuring focus on any potential projectiles, exclusion zone requirements and specific operating conditions according to the OEM
